Click on the form you want to print. A new window will open
with the selected form.

Click on the printer icon on your web browser after the form
appears on your screen.

The print properties box should appear. Please make sure
that the "fit to page" option is not selected.

4)

Click "OK" to print the form.
|
|
5)
|
Close the window with the form.
|

You will need to have
Adobe's Acrobat Reader, Version 9.0 installed on your computer to view and
print the claim forms. Click on the Adobe "Get Acrobat
Reader" icon to the left to obtain the free Adobe Acrobat
Reader file.
