Now What Do We Do?
In order for you and your employees to
begin saving tax dollars with a ADMIN AMERICA POP, complete an
application (detailed instructions are provided
on the reverse side of the single page application) and send
the completed POP Application along with a check for $500
made payable to ADMIN AMERICA at P.O. Box 1810,
Roswell, GA 30077. Your Plan will be delivered within ten
working days upon receipt by ADMIN AMERICA.
You will need to have
Adobe's Acrobat Reader, Version 6.0 or greater installed on your computer to view and
print the claim forms. Click on the Adobe "Get Acrobat
Reader" icon to the left to obtain the free Adobe Acrobat
Reader file.
